Once you have submitted this form, an entry will be created in our database for your appointment. We run tests every Wednesday 4 - 7 p.m. and every Friday 9:30 a.m. - 12:30 p.m. Please click on the date below to book your test. The final testing date for this year is December 16, 2011 (Friday). Testing will resume on January 11, 2012 (Wednesday).
